Student Athlete Medical Forms

We as an athletic training staff hope that you are enjoying your summer vacation! However, the upcoming athletic year is rapidly approaching!

Incoming Student-Athletes

Student-athletes are required to have a full physical examination the first year they participate in intercollegiate athletics at Saint Mary's College. Saint Mary's College Team Physicians, in collaboration with the Athletic Training Staff, will be performing ALL student-athlete Medical Evaluations. These examinations will occur when the student-athlete arrives on campus, prior to the start of their team's participation. At that time, all relevant medical information will be gathered and medical eligibility will be determined. Please be advised that student-athletes WILL NOT BE ALLOWED TO PARTICIPATE without a completed Medical Evaluation. Please be aware, when filling out the Medical Evaluation Form, student athletes need ONLY to fill out the previous medical history portion of the form (Part I), DO NOT complete Part II. This will be completed by our Team Physicians at the time of the medical evaluation.

In addition to the medical evaluation from, we ask that all student athletes complete and return the forms referenced below.

Returning Student Athletes

Please be aware that we are no longer requiring returning student-athletes to have a full Medical Evaluation every year they compete. Instead, we are requiring that they complete and update their physical condition with the Health History Questionnaire referenced below. This form will need to be completed each year prior to Medical Eligibility being granted. Please be advised that your student-athlete WILL NOT BE ALLOWED TO PARTICIPATE without completing this form. Additionally, we ask that all student athletes complete and return the forms referenced below.

We ask that you completely fill out all the information truthfully and to the best of your knowledge. Then return them to us via fax at 925.247.0362, or by mail PRIOR TO JULY 1st . Medical evaluations on any outside forms will not be accepted.
